Patient Reports

Exporting reports out of the GatherMed Platform can be done on an individual level or group level. An individual report will have more details than a group report. Exporting a group report will have the necessary information to bill for all patients in a cycle.

• Pull group reports
• Filter date ranges for reports

Group Report

A Group Report can be used for different reasons, such as, end of month billing. Follow these steps to pull a Group Report:

• Select the Patients Dashboard
• Use the Reading Filters section to select the correct date range
  • Today
  • Week
  • Month
  • Year
  • Custom
• Once you have selected the correct date range, click the Export Data button at the top right. 
  

  

• A Microsoft Excel CSV file will download to your computer with the following information for each patient.
  • Patient Name
  • MRN
  • Sex
  • Date of Birth
  • Age
  • Consent Recorded
  • Consent Recorded Date
  • Start Date
  • Days Since Start
  • Days in Period (selected date range above)
  • Transmission Days (in selected date range)
  • Transmissions (in selected date range)
  • RPM Time
  • CCM Time
  • TCM Time
  • Total Time

Individual Reports

To pull the information on a single patient, use the Individual Reports option. Follow these steps to pull an Individual Report

• Go to the Patient Profile for the patient you want to pull the report
• Select the Export Report button on the top right menu

• 

• This will bring up the Report Filters page
• Select the correct date range
  

  

• Scroll to the bottom of the Report and select Print Report
• A PDF version of the Individual Report will download to your computer
