Register GatherMed Patient Profile

Use this article when first creating your patient profile in the GatherMed RPM platform. 

Step 1 - Visit

  • On the main page, select Register as a Patient 
  • Fill out the Patient Registration information 
    • First Name
    • Last Name 
    • Email 
    • Password
    • Verify Password
    • Patient Code***

A patient code is required to create a patient profile. The patient code ties your patient profile to your provider. Your provider must provide this patient code by email before you can register an account*** Provider instructions can be found here. 

Step 2 - Update Contact Information 

  • Once you have successfully created your Patient Profile, select the Gear Icon at the top right

  • This will bring up the settings page where you can update:
    • Patient Details 
    • Mailing Address
    • Contact Methods
  • Patient Details 
    • Use this section to update your name, date of birth, etc. 
    • SSN and/or MRN are not required*
  • Mailing Address
    • Please make sure the address information is correct
    • The address listed is where GatherMed will ship new devices 
  • Contact Methods 
    • This is the main communication channel between the patient, GatherMed and your provider
    • Compliance and medical alerts will be sent through these channels 
    • Please make sure there are two forms of communication 
    • Make sure to turn the Alert Button to Green
    • The communication options include: 
      • Cell 
      • Phone
      • Email 
      • Fax 
      • Pager
  • Click Save to complete your updates
