Record Patient Consent

In order to enroll a patient into the GatherMed Platform, they must first be consented by the provider. This process can be done outside of the platform. To capture Patient Consent inside the platform, use the following steps:

  • Find the Patient Profile for the patient you wish to consent
  • Look for the Record Consent button at the center of the Patient Profile
  • Select Record Consent
  • An electronic Patient Consent form will pull up
  • It will display your logo if it was added under Settings>Groups>logo
  • Review the form with the patient
  • If patient has consented, select the Record Consent button
