Add Patients

In this section, you will learn how to add patients to the GatherMed platform. Adding patients is a simple, but important, process. It is imperative that you make sure all information is correct when creating a patient profile.

After completing this learning guide, you will be able to:

  • Add new patients
  • Add or update contact methods
  • Have the option to ship devices directly to patient

Add Patient 

Once you sign in to the GatherMed platform, navigate to the Patients Dashboard and select the Add Patient button in the top right. 

This will take you to the New Patient page. This page is divided into four sections:

  • Supervising Provider - the Qualified Health Professional in charge of the patient
  • Patient Information
  • Contact Methods
  • Addresses

From the drop-down menu, select the appropriate Supervising Provider for the new patient. If you don’t see the correct provider in the drop-down, it is likely this person has not been added to the system. You will need to invite them to the system from the Settings menu. 

Patient Information is the next section and includes both required and optional information. The required information to create a new patient:

  • First Name
  • Last Name
  • Date of Birth 
  • Sex

The other information in this section is optional and is left to the discretion of the provider. 

In the Contact Methods section, select the Plus Icon to add a new patient contact method. 

The available Contact Methods:

  • Phone (landline)
  • Cell
  • Email 
  • Fax
  • Pager

There is no limit to the number of contact methods for a patient. The contact methods will be used for Compliance and Medical Alerts. If you want a patient to receive alerts, select the Alert Checkbox next to the Contact Method

The last section to set up the Patient Profile is the Addresses section. This section is important because this is where new devices will be shipped for that patient if they are not sent to the office. It is recommended that you double-check the accuracy of this Addresses information before completing the Patient Profile. If this information is incorrect, the device will not be shipped to the patient. 

Once you have checked all the patient information is correct, select the Create Patient to complete the Patient Profile set up.
