Patient Encounters

It is important that you log every Encounter you have with a Patient Profile. Logging Encounters lets you track how much time you spend each billing period interacting with a patient. This time is used to bill insurance for reimbursement at the end of each billing period. 

 After completing this learning guide, you will be able to:

  • Log an automatic encounter 
  • Log a manual encounter 
  • Save patient notes to encounters 
  • Manual encounter vs automatic encounter

Log Automatic Encounters

Automatic Encounters occur in the GatherMed Platform by clicking the Clock Icon. The platform will record your encounter with the Patient Profile until you are ready to complete the interaction. 

There are multiple ways to start an Automatic Encounter:

  • Select the Clock Icon on the menu at the top right corner
  • Select the Clock Icon next to a Patient Reading on the Readings Dashboard

Select the Clock Icon next to Assigned Patient on the Device Management Dashboard

Once you are ready to complete the encounter: 

  • Double-check the correct patient is listed in the encounter
  • Select Encounter Type at the bottom of the screen
  • Select the Check Box Icon at the bottom of the screen

  • This brings up the Live Encounter page that has the following options:
    • Encounter Patient
    • Encounter Provider
    • Encounter Type 
      • RPM (Remote Patient Monitoring)
      • CCM (Chronic Care Management)
      • TCM (Transitional Care Management) 
    • Encounter Reason 
      • Manage Care 
      • Onboarding - initial enrollment, device orientation, etc. 
      • Monitor Adherence - checking compliance
      • Call Patient
      • Telehealth Visit
      • Order New Device
    • Encounter Start Date/Time
    • Encounter End Date/Time
    • Notes
  • Select all the options except Encounter Start Date/Time & Encounter End Date/Time
    • These are automatically calculated with Automatic Encounters
  • Select the Plus Icon next to Notes
    • Note Type
      • General
      • Recommendation 
      • Reminder
    • Notes
      • Use this to type in any notes you want to capture for this interaction. 
    • Click Save Note
    • Double-check everything is accurate
    • Click Save Encounter

Once the Automatic Encounter has been saved, it will show up under the Encounters tab. 

Log Manual Encounters

Providers are rarely at their desk with the GatherMed Platform open when they are interacting with a patient. We have added Manual Encounters to allow providers to capture a patient interaction after it has taken place. 

Use the following steps to create a Manual Encounter:

  • Find the patient using the Patients Dashboard
  • Select the Patient Profile
  • Select the Encounters tab

  • Click Record Encounter
  • This brings up the Record Encounter page that has the following options:
    • Encounter Patient
    • Encounter Provider
    • Encounter Type 
      • RPM (Remote Patient Monitoring)
      • CCM (Chronic Care Management)
      • TCM (Transitional Care Management) 
    • Encounter Reason 
      • Manage Care 
      • Onboarding - initial enrollment, device orientation, etc. 
      • Monitor Adherence - checking compliance

    • Encounter Start Date/Time
    • Encounter End Date/Time
    • Notes
  • The difference between logging a Manual Encounter and an Automatic Encounter is the Encounter Start Date/Time & Encounter End Date/Time
  • In order for a Manual Encounter to be completed, the following must be done:
    • Encounter Start Date/Time
    • Encounter End Date/Time
    • You must select the Date, Hour, Minute and Second in both options
    • Below is an example of how logging a 10 minute encounter should look*** 

  • Select the Plus Icon next to Notes
    • Note Type
      • General
      • Recommendation 
      • Reminder
    • Notes
      • Use this to type in any notes you want to capture for this interaction. 
    • Click Save Note
    • Double-check everything is accurate
    • Click Save Encounter
